Recovery Support
REHAB uses an interdisciplinary team approach to provide stroke rehabilitation patients with care plans to achieve optimal results. This team consists of the physiatrist (rehabilitation doctor), physical therapist, occupational therapist, speech & language pathologist, recreational therapist, nurse, care coordinator, neuropsychologist and dietitian, who all provide one-to-one therapy and care. This includes:
- Program oversight by Medical Director
- Open-ended communication
- Risk factor and lifestyle modifications
- Individual patient treatment plans
- Discharge summary to physicians
Caregiver Training
Caregivers and family members are provided with a three-hour Caregiver Education and Training (CET) prior to discharge.
