Outpatient Clinic Appointments

To schedule an appointment at one of REHAB’s outpatient clinics, physicians can fax a prescription to the patient’s desired clinic location. Upon receipt, patient will be contacted directly to schedule an appointment. Patients may contact their desired clinic to schedule an appointment directly once a prescription has been received.

Please bring the following to your first appointment:

  • Current picture I.D.
  • Insurance cards
  • Prescription for therapy (if in possession)
  • List of current medications (if applicable)
  • Reading glasses and hearing aid (if applicable)
  • Comfortable clothing and footwear
  • REHAB forms that were sent to be completed and signed before the appointment
  • Co-payment or co-insurance payment

For Specialty Programs:

  • Driver’s Program - current driver’s license
  • Aqua Therapy Program - swimming attire (t-shirt and a light pair of shorts are acceptable)
